Sanctioning Forms
Read through the sanctioning guidelines before completing the forms.  To apply for sanctioning, the event organizer must complete the sanctioning application below and return it along with the deposit to the UPA.

Need help completing these forms? 
  • What do I put on these Insurance Request Forms?  See information and tips for completing the Insurance Request Forms.
  • All electronic forms are word documents.  Simply open the document and complete it by tabbing through the form.  Save the completed document to your computer and email it to Sanction_event@hq.upa.org as an attachment.
  • To download the manual forms, use a computer that is connected to a printer, the Internet, and has Adobe Acrobat.
  • Go to Team Registration for forms that teams and players can use to register for a sanctioned event.
