Registration
If you have been awarded a bid to the 2004
UPA High School Championships, congratulations! This tournament
promises
to be an exciting and unforgettable weekend for you
and your team. But before the fun begins, it is important that we take
care of some pre-tournament business. Please read the instructions
below. All
teams are required to complete all administrative paperwork
completely and on-time
before they will be allowed to participate in the tournament. If you have

Please refer to the Player
and Team Guidelines to
make sure your players and teams comply with all UPA eligibility rules.
Due
Date
All paperwork and fees must ARRIVE (not just postmarked) at
UPA Headquarters by Friday, April 23rd.

Tournament Fees
$20/player - Checks should be made out
to "UPA". Please note on the
check that this is the "High School Championships tourney fee".
Paperwork
All required paperwork can be found
online along
with general instructions. See below for additional descriptions
of the forms found on the forms web page.
- Team Info Form -
The Team
Info Form must be completed by each attending team
and be returned to the UPA along with your roster by April
23rd.
- Roster -
All players attending the tournament must be listed on
an official 2004
UPA Junior Championships roster.
- UPA Membership
Dues -
All players attending the tournament must be UPA members.
UPA dues for Youth Memberships
for 2004 are $20/person.

card by following the instructions on the UPA
website.*

- Waivers -
All players must have a valid waiver on file with the UPA.
Waivers are good for
the calendar
year
in which
they were
signed.
The same waiver
is good for minors and adults (>18),
but minor waivers must also be signed by
a legal
guardian.
- Medical Release Form -
All minors (players under 18) must return a completed medical
release form.
This form
empowers
a person or
group to authorize
medical care for a minor while at the
tournament.
- Chaperone Waiver -
Each attending team must have 2 chaperones who are at least
25 years
old. All
chaperones must fill
out and return
a chaperone
waiver.
