Congratulations on earning a bid to the 2004 UPA Club Championships! This tournament promises to be an exciting and unforgettable weekend for you and your team. But before the fun begins, it is important that we take care of some pre-tournament business. Please read the instructions below. All teams are required to complete all administrative paperwork completely and on time before they will be allowed to participate in the tournament. Registration Information
You or your team captain should have received a Qualifier’ Packet from your Regional Coordinator following Regionals.  The Qualifier’s Packet contains complete registration details.  Please contact your regional coordinator or UPA HQ if you did not receive this information.  A few important registration items are also outlined below.

Pre-registration must be done via email by Friday, Oct 15th. See paperwork details below.

Registration at the event will take place on Wednesday, Oct 27th at the Holiday Inn Lakewood Ranch right outside the Osprey Room from 6-8PM. UPA staff will be on hand to check teams in, take care of any outstanding paperwork, and give out player packs. All teams should check in at registration prior to the captain's meeting. All paperwork issues MUST be taken care of by Wednesday night or the team/players in question will not be allowed to play.

Team info due at UPA HQ by October 15th. Tournament fee due at HQ by October 22nd ($50 late fee).

  • Tournament fee - $45 per player (check made out to the UPA)
  • Team info – Save the Team Info Form (doc) to your computer, fill it out completely, and email it to Michael Degnan no later than Oct 15th.

Captain's Meeting
The captain's meeting will be held Wednesday, Oct 27th at 8PM at the Osprey Room at the Holiday Inn Lakewood Ranch.  Attendance is mandatory.  Tournament info, competition issues, and any last minute changes to pre-tournament info will be discussed.
